Concierge Physician Associates strives to provide personal care for each of our patients in order to provide the optimum care and allow them to maintain their health. Concierge Physician Associates provides care in our office for our patients and also helps coordinate care for our patients with external specialists and hospitals should the need arise.
We understand that every patients needs are unique to them. Although our patients medical needs may vary, Concierge Physician Associates prides ourselves in providing the same high-level of care to all of our patients.
Concierge Physician Associates feels that a high-level of patient care includes being seen by a physician when you feel you need to be seen. We strive to get our patients scheduled into our office the day or day after they call requesting to be seen for urgent medical concerns including minor injuries, colds and illness that are not an emergency.
Concierge Physician Associates Provides The Following Services For A Monthly Fee
Urgent Care
If you are not feeling well or feel you need medical attention we will provide a same day or next-day evaluation in order to diagnose and begin treating minor illness or injury. Should Concierge Physician Associates feel that your condition requires emergency care or review by a specialist we will assist you in coordinating the appropriate care.
Preventive Care
We understand that our patients lifestyles, age and health vary. We will work one-on-one with our patients to develop a medical plan that is specific to them as an individual. Age appropriate screenings and evaluations are completed for all of our patients to assure a proper assessment of their health.
Wellness Care
Concierge Physician Associates feels it is important to provide wellness care to our patients that improve their overall health. We will provide our patients with consultation and coaching on common issues that effect health including smoking, obesity and stress.
Chronic Disease Management
National guidelines regarding chronic diseases are carefully followed by Concierge Physician Associates. Patients suffering from heart disease, hypertension, diabetes, asthma, arthritis, osteoporosis, and other conditions are met with state-of-the-art care. We will also provide counseling to assist you in managing your condition. Should Concierge Physician Associates feel that your condition requires specialized care outside of what we can provide, we will gladly assist you in choosing a provider and guide you should you have questions regarding their recommendations.
Specialist Care Coordination
Occasionally our patients will present with a medical condition that requires specialized care outside of our practice. Concierge Physician Associates will assist you in finding a specialist to meet your medical and financial needs by referring a specialist that is covered by your insurance plan. If you do not have insurance we will refer you to a provider that offers discounted rates to patients paying out of pocket. We will then brief them on your medical condition and follow up with your specialist after your consultation.
Hospital Care Coordination
Concierge Physician Associates cares for our patients and if hospitalization is required we will work with the hospital to personally review and supervise your care. Everything from your admission to the coordination of physicians, specialists, hospitalists, charts, nurses, procedures, lab work, radiology, pathology and discharge will be carefully reviewed by your physician with Concierge Physician Associates.
After Hours Coverage
Occasionally our patients have questions or concerns about their health after our scheduled practice hours. Should you need to contact us after hours, your personal Concierge Physician Associates physician will be available to answer your questions. Calls after-hours on the weekend, holidays or during a scheduled vacation are returned by the on-call Concierge Physician Associates physician.
Office Procedures
Concierge Physician Associates offers diagnostic and urgent care procedures such as EKGs, PAP smears and spirometry at no additional cost to you. If a procedure requires additional payment for material or lab analysis you will only be charged for the cost of these items.
Medications & Vaccinations
Certain medications and vaccinations are available to in our office to our patients at our cost.
